To cancel you must contact PFT via text or call.

I hereby grant permission to PFT to use my child's image on its website and social media without further consideration. I understand that complete names of minors will not be used.​

I hereby give permission to my to my child to participate in the PFT training and workouts. I further waive, release, absolve, indemnify and agree to hold harmless the coaches and staff of PFT from any responsibility for injury before, during or after any activity. It is understood that participation in these workouts may result in injury and that protective equipment does not prevent all injuries to participants. In case of a medical emergency, and a parent cannot be reached, coaching staff has my permission to obtain treatment.
